Our PCN operates across Northumberland, serving tens of thousands of patients through a network of GP practices. By working together, we ensure patients receive the right care, at the right time, from the right healthcare professional.

What our PCN offers

We integrate a skilled multidisciplinary team, including clinical pharmacists, physiotherapists, paramedics, social prescribers, and mental health practitioners, supporting GPs and improving patient care.

Our PCN is designed around the specific health needs of Northumberland’s communities, ensuring care is personalised and responsive to local populations.

We focus on preventative medicine, offering vaccination programs, health checks, and long-term condition management to keep our communities healthier for longer.
