How to register

Registering with our practice is quick and easy. You can download a registration form by clicking the button below. Please fill out this form and send it to the practice via elsdon.avenuesurgery@nhs.net.

To register, you must also send a copy of your ID. This can be a passport, driving licence, utility bill or medical card. Children do not need to do this as long as at least one parent is registered with the practice.

Please also complete and return a copy of the patient survey. You can download this by clicking the button below.

If you are unable to print or e-mail the documents, please collect a paper copy from the site where you wish to be registered.
